This form must be completed and all work must have UGLPOA Parklands Committee approval prior to the start of any project or repair. Call the UGLPOA office at 973-853-4262 to obtain an appointment for a site review. Pictures of the area to be worked on must accompany this form. Some projects/repairs will require a drawn plan in addition to the pictures. The completed approved form must be on file in the UGLPOA Office prior to the start of any project.
